Warped! Same thing happened to a new Olds Alero my wife had after only 6 months from the lot! It was so bad it was like the steering wheel had seizures. Turned out they were warped and needed replacement, resurface would not fix. Just brakes has a 4 wheel friction reline 4 $99.99

Replace pads and shoes on all 4 wheels
Machine Rotors & Drums
Pack the bearings
Adjust & bleed the brakes
Inspect the balance of the brake system.

Thats alot of work for $99.99 / The toyota dealer charged me $149.99 for just the front pads, resurface, & adjustment. Well worth going to this place!

Your case might be differ cause a replacement of the rotors is more than likely required since you said it felt like it was falling apart. I know that feeling. You might was well get OEM size gas slotted rotors from brembo. This is what I would do: I would change the rotors myself and then take it in for brake tune up.

Do you have rear drums or disks (if you have a 4 cyl - most likely you have drums)? Oddly enough, bad rear drums can cause a lot of vibration that feels as if coming from the front. I had this on my Camry, and a friend still has it on his - dealer said in both cases, the fronts needed resurfacing.

Got new disks and pads - same thing. Eventually I figured it was the rear drums causing the shaking and vibration. I replaced my drum brakes with a complete rear disk brake setup (not exactly trivial) and the vibrations are gone :) . My friend does not care that much so after resurfacing and new pads in the front, and about $100 for this - he still drives with vibrations...

I'm not saying that this will fix YOUR vibration - it fixed mine... It cost me just under $300 USD for parts (actually - about $200 for the parts from junk yard plus resurfacing the rotors, brake fluid, paint, etc.)

You need to first diagnose if this is your issue too: drive on an empty road at 50-70 mph and pull the parking brake hard. Make sure you keep the release button depressed in case your wheels lock. When I did this test - my car vibrated much the same as when I applied the brakes under normal conditions. Thry this a few times after your brakes have warmed-up to see if yours vibrate. If it does - there is your problem. If not - perhaps your front brakes are in need of service :)
